| To enroll a student in the SPARK program, a parent or guardian must fill out an application form. A completed enrollment form must be received before the first day of attendance. An enrollment form includes:
- medical emergency information
- family information
- parent agreement questions
Click on a copy of the application package below. Print the forms, fill them out by hand, and return them to your SPARK staff.
You may also pick up a SPARK application at your school office.
Enrollment is not on a first-come, first-served basis.
