| To enroll a student in the SPARK program, a parent or guardian must fill out application forms. A completed enrollment packet must be received before the first day of attendance. An enrollment packet includes: - enrollment form
- medical emergency form
- family information
- parent agreement forms
Click on a copy of the application package below. Print the forms, fill them out by hand, and return them to your school office. You may also pick up a SPARK application at your school office.
Enrollment is on a first-come, first-served basis.
